Blood of Frahagil
Potion, legendary
Among the many organs and remains of Frahagil, the Immortal Juggernaut, its blood is among the most sought-after commodities on the black market. It carries the creature’s unparalleled regenerative properties, capable of mending grievous wounds, curing diseases, restoring severed limbs, and even returning the recently dead to life. Some claim it can even slow the passage of time within the body and extend one’s lifespan. Yet, like all parts of Frahagil, the blood is steeped in its relentless rage and unnatural resilience. Those who consume it risk being overwhelmed by its violent vitality, at best losing control of themselves for a time, and at worst being twisted into a monstrous reflection of the creature itself. The blood is also notoriously difficult to contain, requiring complex measures to prevent it from regenerating into a living, hostile mass.
Those who seek to acquire it must exercise caution, as diluted imitations are often passed off as the real thing. While these lesser concoctions retain some of the blood’s restorative properties, their effects are diminished, and their instability far more immediate and volatile.
This heavy glass bottle is sealed with layered sigils that pulse faintly like a heartbeat. Within, a measure of deep crimson blood coils and reforms endlessly, pressing against the glass with unnatural intent and making the bottle unnaturally warm to the touch. Once the seal is broken, the blood rapidly regenerates and begins to overflow. At the end of 1 minute, if it has not been consumed or contained, it spills free and becomes a hostile creature using the statistics of a black pudding, which attacks indiscriminately.
If consumed by a living creature. The creature regains 100 hit points, is cured of all diseases and poisons, and regrows any missing body parts.
A creature that benefits from this potion must succeed on a DC 18 Wisdom saving throw or enter a berserk state for 1 minute. While berserk, the creature must use its movement to approach and its action to attack a creature it can see, choosing the nearest target if possible. It has advantage on melee attack rolls and deals an extra 2d6 damage on a hit. At the end of each of its turns, the creature repeats the saving throw, tracking successes as with death saving throws. On a success, the creature regains control until the end of its next turn. The effect ends after three cumulative successes or when the duration expires.
If consumed by a dead creature. As an action, a creature can administer the contents of the bottle to a creature that has been dead for no longer than 1 hour. The creature returns to life, regains 100 hit points, and regrows any missing body parts.
A creature revived in this way must make a DC 18 Constitution saving throw at the end of each of its turns, tracking successes and failures as with death saving throws. On three successes, it stabilizes and retains its consciousness. On three failures, it transforms into a monstrous version of itself, using the statistics of a troll while retaining its ability scores (if higher) and proficiencies. It cannot cast spells or concentrate on them and loses all other class features. It is hostile to all creatures and is consumed by violent rage. While in this state, it must use its movement to approach and its action to attack a creature it can see, choosing the nearest target if possible. This rage lasts for 1 minute. After this duration, the creature regains its sentience but remains in its transformed state.
The transformation persists indefinitely. At the end of each long rest, the creature can make a DC 18 Wisdom saving throw. After three cumulative successes, the creature reverts to its original form. A wish spell or similar magic can also end the transformation.
Lingering Mutation. A creature affected by this potion undergoes permanent, visibly apparent physical alterations as the blood reshapes its flesh. The exact form of these mutations and whether they carry mechanical effects is left to the DM’s discretion.
(Diluted) Blood of FrahagilPotion, rare

Once the seal is broken, the contents overflow and rapidly rot within 1 minute.
Vital Surge. A creature that drinks this potion regains 60 hit points, is cured of diseases and poisons, and regrows one missing limb or organ after 1 minute.
Unstable Mutation. Immediately after drinking, the creature must succeed on a DC 15 Constitution saving throw or begin to mutate. While mutating, at the end of each of its turns and the first time it takes damage on a turn, it repeats the saving throw, tracking successes and failures as with death saving throws.
On a failed save, the creature is consumed by violent rage until the end of its next turn. While enraged, it is hostile to all creatures and must use its movement to approach and its action to attack a creature it can see, choosing the nearest target. If multiple creatures are equally near, it chooses one at random.
The effect ends after three successes. On three failures, the creature transforms into a mindless monstrosity for 1 hour. Its game statistics are replaced by those of a troll, and it loses all class features, proficiencies, and memories. The creature is hostile to all creatures and acts according to its nature. When the transformation ends, the creature reverts to its original form and gains 1 level of exhaustion.
Lingering Mutation. A creature that drinks this potion bears minor, visible mutations at the DM’s discretion. The exact form of these mutations and whether they carry mechanical effects is left to the DM’s discretion.
