Our game's protagonist often gets himself involved in circumstances that are a tad too dangerous for him, and although he always escapes with his life, it's not uncommon for him to suffer a few bruises along the way. Something similar happens in Gomira's story, and in this image, you see him sprawled on the ground after being hit in the face.
