How many 🍆 does true form Sukuna have?

I mean the form with the extra eyes and arms and mouth, NOT the "modern" version I draw sometimes (with the same build but regular number of limbs + scars instead of the extras). 

I need it for science.

I have my own ideas but I'm curious about YOURS!
