In CAS give your Sim the Secret Lover aspiration, located in the Intimate category.
The bonus trait is "Sneaky", this trait will give a higher boost in the romance track.

Give your Sim the Secret Lover trait from the lifestyle category.

The Secret Lovers menu will show up on Sims you've haven't create a secret with like this:
The Sneaky menu is a friendly menu to help you keep your relationship under wraps. It'll have excuses and lies to hide.
Just Between Us will have a couple of interactions to allow you to create the secret affair with the Sim you have a relationship with. This menu changes after the agreement is made.
Make sure you have some level of romance with the Sim or neither will appear.
There's a chance the Sim you're asking will not agree to this.
This is the menu that will appear on the Sim you've made the agreement with.
Meet In Secret is similar to the date interaction. It'll launch an event, select your household or community lot to travel to. Goal the event. Only gold will be successful.
The main goal may require you to use a couple more friendly social interactions like get to know or discuss interests to complete 10/10
End the event.
I updated the requirement to meet in secret from 4 to 1.
Discreet Operator is the reward trait for the aspiration.
To end the secret about the romance, click on the romantic partner, action menu, select Breakup Secret Romance. This will remove the hidden trait from the partner Sim. The relationship will be the same.
